Request a Transcript

There are two ways to request an official copy of your YES Prep Public Schools Transcript. 

1. Contact your secondary campus front office to make a request.

OR

2. If you are a YES Prep Eisenhower or Northbrook alumnus, please email the below information to alumni@yesprep.org.

• Full Name
• YES Prep Campus Name
• Graduation Year
• Date of Birth
• YES Prep ID Number
• Indicate the method of delivery and recipient of the official Transcript
  • Electronic Copy - Email
  • Physical Copy - USPS Mail Address or personal pickup.
